
Live in Control is a four-week, community-based, diabetes self-management education (DSME) program designed to help individuals with type 2 diabetes better manage their diabetes It's a foundational DSME course that aims to build participant's knowledge and skills to achieve blood glucose control.
What makes Live in Control different from other diabetes self-management programs?
A core philosophy of the program is that a family-centered approach is necessary to better manage diabetes. Live in Control includes a family member or friend (care supporter) of the participant as a part of the intervention. This person is someone who they identify as a person closely involved in their diabetes management.
