
Healthy Heart Ambassador
HHA is an individualized program that teaches people how to self-monitor their blood pressure. Participants attend one-on-one sessions with a trained facilitator to learn proper blood pressure techniques, nutrition education, and lifestyle changes.

- Free 4-month program
- Participants attend two personalized 10-to-15-minute consultations per month
- Participants attend one 60-minute Nutrition Education seminar each month
- Facilitators offer support through weekly emails, text messages, or phone calls.

- Be at least 18 years old or older
- Have been diagnosed with high blood pressure and/or are taking an antihypertensive (blood pressure) medication
- Have NOT experienced a cardiac event within the past 12 months.
- Not have irregular heartbeats (atrial fibrillation or other arrhythmias)
- Not have swelling in the limbs due to abnormal buildup of fluids (such as lymphedema and other conditions, often caused by cancer treatments that remove or damage your lymph nodes)

- You will receive a free blood pressure monitor and log to help you learn how to properly measure and record blood pressure readings
- You will receive individualized support, guidance, and encouragement as you learn how to regularly check and record blood pressure
- You will gain an understanding of blood pressure readings and actions to take to maintain a healthy blood pressure
- You will participate in four, one-hour nutrition lessons, which cover information to help you plan, shop, cook, and eat heart-healthy foods and meals for a lifetime.
